About This Resource

This Wellness and Access Form was developed by 2b theatre company in 2022 as part of their Balancing Act Level UP! Pilot Project. The form is set up as a Google Form and is designed to provide all team members at the company space to communicate their personal needs/ preferences, working styles, and more. The form is updated on an ongoing basis to reflect the evolving needs of artists and arts workers.
