Buddies in Bad Times Theatre created this Reimbursement Form in 2022 as part of their Balancing Act Level UP! Pilot Project. Artists and arts workers were asked to fill out this form and submit it along with any receipts/invoices in order to be reimbursed for care-related expenses.
