Canadian Alliance of Dance Artists – East Chapter (CADA/East) created this Caregiver Subsidy Receipt Submission Form in 2022 as part of their Balancing Act Level UP! Pilot Project. CADA/East members who participated in the Caregiver Subsidy Program were asked to fill out the form each time they wanted to request reimbursement for caregiving expenses related to their dance work.
