Caracol (Carole Facal) created this Retreat Application Form as part of her Level UP! Pilot Project. The form was designed for individuals interested in attending Caracol’s Sound Therapy & Creativity Retreat for Artist-Caregivers in May 2025. It was created using Google Forms and made available through the retreat webpage and via direct outreach to interested participants online.
