About This Resource

Monica Lacey created this Participation Waiver in 2025 for participants at her Creative Retreat for Artist-Parents and Their Children. The retreat was the central focus of her Level UP! Pilot Project. Adult participants completed the waiver for themselves and any children in their care who accompanied them at the retreat.
