Toronto Dance Theatre created the following Caregiver Needs Assessment Survey in 2022 as part of their Balancing Act Level UP! Pilot Project. The original survey was created as a Google Form and sent to Toronto Dance Theatre employees to increase understanding of caregiver needs at the company. Survey responses helped to inform the development of a Caregiver Support Pilot Program.
