About This Resource

This Childminding Intake Form was developed in 2022 by Common Boots Theatre and Two Birds Theatre as part of their Balancing Act Level UP! Pilot Project. The form was created as a Google Form to invite families to sign up for free childminding/children’s arts workshop during performances of Apocalypse Play. The form was shared as part of marketing for the play to encourage people to register in advance.
