Quote Unquote Collective created this Care and Access Needs Questionnaire as part of their Balancing Act Level UP! Pilot Project. The questionnaire was shared with artists participating in Quote Unquote ’s Universal Child Care workshop in 2021 to assess care and access needs of participating artists.
